The 2024 Let's Talk Health, Chatham-Kent! Survey is now open!
The Chatham-Kent Ontario Health Team (CKOHT) is a group of local health and community organizations working together to plan, design and improve the local healthcare system.
The ultimate goal of the CKOHT is working together with health and community partners from all sectors, including hospital, home and community care, primary care, long-term care and community support agencies to provide seamless, fully coordinated care for patients, while creating a more efficient system.
We Want to Hear from You!
Your experiences accessing local health care services;
Opportunities for improvements in system navigation and digital health; and
Priorities the CKOHT should focus on in Chatham-Kent.
