The Chatham-Kent Ontario Health Team (CKOHT) is a group of health care organizations working together to plan, design and make changes to our local health care system with the purpose of better access to care.

The ultimate goal of the CKOHT is working together with health care partners from all sectors, including hospital, home and community care, primary care, long-term care and community support agencies to provide seamless, fully coordinated care for patients, while creating a more efficient system.

CKOHT’s vision is: “Achieving the best health and well-being together.”

We want to hear about:

  • your experiences accessing local health care services;
  • if and how the COVID-19 pandemic has affected your health; and
  • what your priorities are for health care in Chatham-Kent.

Your answers will help plan for health care in the future.

    Purpose of the survey:

    Thank you for taking the time to fill out this survey. This survey will take you about 10 - 15 minutes to complete. It will ask you questions about health care in Chatham-Kent. The answers will help us to plan health care services in our local community. Le sondage contient des questions concernant les soins de santé dispensés dans la municipalité de Chatham Kent. Vos réponses nous aideront à planifier les services de santé offerts dans notre collectivité. Si vous ne vous sentez pas à l’aise de répondre à une question, vous pouvez ne pas y répondre et passer à la question suivante.
